Coverage:

Empire Plan NYSHIP covers up to 20 Medical Massage sessions for pain and injury for NY State employees.

Required Documentation:

1. Prescription from your healthcare provider (Primary Medical Doctor, Orthopedist, Neurologist, DO, or Chiropractor
2. Prescription should include: Name, date of birth, diagnosis code (low back pain/muscle spasm), prescribed treatment (massage therapy), frequency (2-4 times per week/month), and physicians name address and license number
3. Copy of insurance card
